About asthma management in SFUSD schools

 

Asthma is a leading cause of school absenteeism. With proper asthma management and making sure our children have healthy learning environments, we can all help to keep children with asthma in school, where they will grow and learn with their peers. Healthy Schools also keep our children's teachers and other staff healthy.

Asthma Management and Emergency Care Instructions:

If your child has asthma and attends SFUSD K-12 make sure to complete these required forms with your medical provider and return them to your school. You must file these forms again every school year.


1. SFUSD Medication Form is required each school year to allow for your child to carry and use their asthma medicine at school.

2. SFUSD Asthma Emergency Care Plan must be filed each school year for all SFUSD students with asthma in K-12th grades. These plans tell school staff about your child's specific asthma triggers, medications, and what to do in an emergency.

3. If your child is in the SFUSD Child Development Program you must contact your site manager directly to discuss asthma or any other healthcare needs.
