LEAGUE PLAY

Youth league will run on Sundays and will start January 3, 2010.  
Adult league will run on Monday nights, and will start January 4, 2010.
The league will run through March 22, 2010 (10 weeks)

THE LEAGUE WILL GIVE ONE BY TO TEAMS THAT REQUEST IT AT TIME OF REGISTRATION AND THAT PARTICIPATE IN THE FOLLOWING 2010 CUPS: STATE CUP, ASSOCIATION CUP AND FOUNDERS CUP 
NO U.S. CLUB CUPS OR ANY OTHER SECTIONAL CUP WILL BE GRANTED BYS.   FUTSAL® COMES FIRST!


Game sites include: Upper Noe Rec Center, Mission Rec Center, St Mary's Rec Center, Glen Park Rec Center, Eureka Valley Rec Center, 
Minnie & Lovie Rec Center, SOMA Rec Center, Palega Rec Center, Joe Lee Rec Center and Moscone Rec Center. 

There will be no games on: Sunday Jan 17, or Monday Jan 18, 2010
due to Marting Luther King Day


There will be no games on:
Sunday February 14 and Monday February 15, 2010 due to President's Day Holiday

Day light savings starts on Sunday March 14, 2010.  Clocks are set ahead one hour!
 

YOUTH DIVISIONS

no youth player is allowed to paticipate with youth and adult teams at the same time

U7, U8, U9, U10, U11, U12, U13, U14, U15, U16, U17, U18  Boys Rec and Premier Divisions.
Please provide us a copy of your fall standings so that we have a better idea in which division to place you if you cannot decide on your own.

(all divisions will have post season games with the exception of the U7 and U8 divisions.  These divisions will receive participation awards for all its players and coaches)

Age cut-off date is August 1, 2010.

ADULT DIVISIONS
(Players need to be 18 yrs. of age or over)

ALL GAMES WILL BE PLAYED AS SCHEDULED. THERE WILL BE NO CHANGES

ENTRY FEE

Youth divisions: $250.00 per team
Adult divisions: $500.00 per team

A separate fee of $5 per youth player and $7 per adult player will be assessed for medical insurance.
This second check should be made out to USFF and turned in when registering.
(i.e. if you have a roster of 10 youth players, you will to write a check for $50
made out to CFAN and you will submit it to the league at the time of registration)

You can pay by company check, money order, cashiers check or credit card
NO CASH!

THERE WILL BE NO REFUNDS AFTER START DATE OF LEAGUE

ENTRIES

The league is limited to nine youth (9) teams per division and seven adult (7) teams per division. Extra teams will be placed on a waiting list. First come first served. Gym space is limited, so register soon!   Each team is limited to a maximum of twelve (12) players on their roster with an absolute minimum of eight (8) players. You can also have a maximun number of 2 coaches registered.  Additions and deletions to the roster must be done in writing with the league coordinator and must be on file before a player is eligible to participate. Futsal Add/Drop forms (PDF) can be downloaded from this website.


CYSA NORTH AND USFF HAVE MERGED, SO ALL YOUTH TEAMS, ARE ALLOWED TO USE CYSA PLAYER AND COACHES PASSES.
ALL CYSA PASSES NEED TO BE SHOWN AT THE TIME OF REGISTRATION OR BY DECEMBER 11, 2009 (END OF REGISTRATION)  IN ORDER TO BE ALLOWED PARTICIPATION IN OUR LEAGUE

PROCEDURE FOR REGISTRATION ONLINE WITH USFF

Step 1. To register online go to www.futsal.org 
everyone needs to click on registration center at the top of the main page or go to  http://www.futsal.org/index.php/registration-center 
Click on Coach Online Registration
from the drop down menu choose returning or new coach.  (If you participated in our league in the past 5 years, you are a returning coach.
If you participated in our league for the first time last year or for the first time this year, you are a new coach.)

Each coach will pay a fee of $12 when registering.  You may register up to 2 coaches. 
CHOOSE CAL NORTH AS THE STATE ASSOCIATION AND JOSE CORONADO AS THE LOCAL LEAGUE...

After you register as a coach, you will be given a coach ID number to register your team

After you register your team(s) you will get a team ID number and a password to register players


AFTER COMPLETING ALL THESE STEPS PRINT OUT A ROSTER

Step 2. Submit roster to Jose Coronado league along with two checks or money orders. 
One for the team fee ($250 for youth or $500 for adult) made out to SF Rec and Park
and the other for the total of the number of players times the amount for either youth or adult fee
for secondary medical insurance ($5 per youth player or $7 per adult player) made out to CFAN.
 

Youth teams: Submit CYSA Player Passes to have them checked.  If you have players that have no CYSA Passes or are new, you must submit one photo and proof of birth. The league will print our USFF player passes for them. 

Adult teams
: WOMEN division: All teams can use their GGWSL player cards.  The league can also print player passes for those without GGWSL player passes but you will need to provide one photo for each player and each coach at time of registration.  If a coach is a player also, her spot in the team will count as one of the twelve permitted.

Adult teams: MEN division: The league will print out USFF player passes but you will need to provide one photo for each player and each coach at time of registration.  If a coach is a player also, his spot in the team will count as one of the twelve permitted.


if you need to erase a player from your roster, you can only do it by asking the federation to do it for you.

JOSE CORONADO LEAGUE REGISTRATION LOCATION AND TIMES

You may turn in or mail your roster and fees to Crocker Amazon Park (799 Moscow St SF 94112 at Italy St) during the following days and times
(Please, place your paperwork in an envelope with your team's name on it and a contact name and phone number)

Monday 1pm-5pm
Tuesdays 1pm-6pm and 7pm-9pm
Wednesdays 1pm-8pm
Thursdays 1pm-6pm and 7pm-9pm

ADDING PLAYERS
 

If you wish to add players to your team roster after you have turned in your paperwork you must first register them online at  www.futsal.org  and then download our add/drop form from this website; fill out completely and turn it in to league coordinator during the above hours. You will also need to submit the CYSA player or coaches pass to have it checked and stamped. The league also lets you transfer players. NO PLAYER  OR COACHES PASS, NO PLAY!!


LOOKING TO JOIN A TEAM?

If you want to join a team in our Futsal® league, you need to give us your full name, date of birth, age, contact phone number and or email address (parent's name too if you are a minor). We will then forward this info to all teams joining our league so interested coaches can contact you. Email all this info to  Luis Azucena at recparksoccer@sfgov.org or call 337-4708.

Release of liability

As a participant in the activities of the City and County of San Francisco Recreation & Park Department, I hereby waive and release any claims I may have, now or in the future, against the City and its officers, employees, contractors, servants and agents (hereafter referred to as The City) arising from injuries to me, or damage to my property, sustained while I am: (1) at The City's facilities, (2) participating in the City's activities, or (3) being transported to or from the City's facilities or activities, regardless of whether such injuries or property damage is caused in whole or in part by the City's active or passive negligence.

In the event of an injury to my child, I hereby give the City permission to arrange transportation for my child to a medical facility, and/or provide my child with emergency treatment or first aid. I understand that the City does not assume any responsibility to take any of these actions.

This waiver and release shall be valid for the duration of the sessions in which my child is enrolled. I have carefully read this waiver and release and agree to the terms stated. By registering with US Futsal, SF Rec and Park and by signing roster form I agree to terms stated.
