CASC Case Management Services

Individuals must be clients of the San Francisco Adult Probation Department to access the below services.

UCSF provides clinical case management services for clients identified as needing mental health services. Clients will receive comprehensive assessment, crisis intervention, case management, individual and group therapy, and substance abuse services.  Services will be provided at Citywide, the Community Assessment and Services Center or in the Community.

Senior Ex-Offender Program

SEOP provides case management services to clients who are 40 years of age or older.  Clients will work with a primary case manager to identify needs, and will receive assistance in addressing barriers to their reentry.  These services might include connecting clients with public assistance, medical, and housing services.

Behavioral Health Services

SFAPD and the Department of Public Health maintain a strong partnership for the purpose of engaging clients in behavioral health services.   Services include one on one clinical therapy, care coordination into substance dependency, and mental health treatment, and  clean and sober transitional housing on Treasure Island.
