As the CASC's main service provider, Citywide provides both clinical and reentry case management services. Clients will receive a comprehensive assessment, crisis intervention, case management, individual and group therapy, and substance abuse services.
Senior Ex-Offender Program
SEOP provides case management services to clients who are 35 years of age or older. Clients will work with a primary case manager to identify needs, and will receive assistance in addressing barriers to their reentry. These services might include connecting clients with public assistance, medical, and housing services.
M3 Case Management
M3 provides case management services to pretrial clients referred by the courts, as well as APD clients who are participants in Intensive Supervision Court (ISC). All clients are assessed, complete an individualized services plan and are enrolled in CASC programming with a special focus on substance dependency and recovery. Other services include housing planning/shelter referral and services brokering.
Behavioral Health Services
APD and the Department of Public Health partner to engage clients in behavioral health services. Services include one on one clinical therapy, as well as care coordination into substance dependency and mental health treatment.
