Review Minutes, Agenda & Set Time

Anjali reminded group that website is live and it contains the asthma management flyers,

 

Nan received a call from a parent that she thinks might have been referred from ATF (likely website).  We’re not sure who made the referral but it was related to a medical question.  Brings up larger question of really reaching the public and knowing how to make referrals for medical questions and how to know how people are hearing about ATF.

Check in with Dorothy Quan regarding how the phone referral line.

Asthma Emergency Care Plan folders

Peg reinforced her interest in revising the Asthma Emergency Care Plan form.  We need some clarification on whether SFUSD is willing to look into revisions for next year or not for a few years.

Marie suggested that DAT would be the forum to reintroduce the idea of altering the Asthma Emergency Care Plans.

Anjali check in with Meyla about who to contact at SFUSD re: Asthma from now on.

Peg is trying to coordinate CMEs and needs everyone speaking to

 

Suggested that Susan Janson, adult NP from UCS, PhD in Nursing, be included in these efforts

 

Suggested specific talking points for speakers:

 

Opening statement:

Reinforce the fact that this is about sharing what’s going on now, we know we all face challenges in our systems and experience successes.  You have to make your system yours and not wait for others or higher ups to make the change.  Even if you just show someone how to use a spacer/peak flow you are doing a tremendous amount.  This is the beginning and we hope that we can continue to work together…

Helping them understand their asthma management

Proactive identification

Cost-effective

5 diseases causes majority of hospitalizations and ER visits  - asthma, hipertention, COPD, CHF, Asthma

Both health plans have resources for bed covers etc.

Purpose of spirometry, why promote it? Why useful? What does it tell you?
