Vaccine Eligibility Attestation Process Available for SF Personal and Home Care Providers

Dear Disability Community Members:

The Mayor's Office on Disability is pleased to announce a vaccine eligibility attestation process for personal, family, and home care providers of people with disabilities and older adults.

As you may know, In Home Supportive Services (IHSS) employees are currently eligible to register for a vaccination appointment.  Other care providers of persons with disabilities and older adults, like private pay and family caregivers, are also eligible under Phase 1A.

This health care worker verification site has been updated to reflect eligibility for all care providers.    Please review the Home Health, Home Care and In-Home Supportive Services section.  Language translation will be available by early next week.

IHSS employees should continue to follow the vaccination process as previously outlined by your employer.

For those who use care support other than IHSS:

Step one:  The person receiving support completes the attestation form (one form per care provider).

Step two:  The care provider registers for a vaccination appointment.

Step three:  The care provider brings the attestation form with them to their vaccination appointment.

At this time, for vaccination appointments are available for those who meet the criteria in Phase 1A only.

Phase 1B is expected to open the week of February 24, 2021.  Those eligible in Phase 1A are encouraged to schedule a vaccination appointment as soon as possible.
