Poor lighting and dreary

Pillars difficult for seating

Try classroom seating in future (Reserved for Oct. 12th), perhaps V shape?

Food:

$26/person breakfast, lunch and dinner

$5000 used for 2 events

q             Food – frittata or yogurt next time?

Logistics:

q     Parking stickers for speakers worked out well

q     Signs with arrows helpful

 

 

 

Attendees:

48 people – not necessarily the same people from past.

Success – UC?  North East Medical Center?

Could head nurses nominate others to be asthma champion?

Limits to how many from each clinic.

PHNs get education leave.

 

 

Survey revealed people did not like brainstorming session. Why?  Wrong people, no lunch break, room too big, not enough framing?

Add brainstorming suggestions to sustainability conversations.

 

q     Anjali – Are there notes from the brainstorming group?  Additional comments from surveys? what % of participants from past events have attended 2+ SFATF events?

q     Anjali – Did people express interest in medical/legal partnerships? Report back to Karen

q     Committee – brainstorm list of people in power to invite to Oct. 12th.

Susan Janson asked what are objectives of task force and noted institutional change is very difficult.

Kaiser Local Community Benefit Grant Application:

p. 5 Missing information needed.  Qualification sheet from Karen needs approval.  (see handout)

Grant will allow funding for on site trainings.

q     Per funding approval, ask Mindy Benson & Elizabeth Edwards to Oct. 12th?

800 hits past year

Karen has been trained and we should be able to maintain without webmaster.  Only city employees can modify site.

Goal:  link existing resources & patients to clinicians.

All resources should be checked with supervisors –

Difficult to meet promises.

Need different resource guide for various clinical setting needs

 

What part of system will direct adults with asthma to care?

Where in health access system will people receive asthma education?

Will E.D.s commit to handing out A, B and C resources to each patient with asthma after the SFATF sunsets?

HEDIS (Health Plan Employer Data Info. Systems) - Evaluations help implement?

Give to Health Commission notes on what has been learned re: need for systems change, barriers for implementation, and future recommendations

CPMC hospital – adult asthma classes well attended. CPMC process;  ER -> Private physicians -> Health Resource Center

Karen received call and they are interested in supporting SFATF

Next step for connecting with CPMC?  $25,000 grant for asthma.

Report card possible questions….

 

·        What do they do after someone is admitted to ER?

·        Do you know which patients have asthma?  How are they identified?

·        How do your patients get spacers, peakflow meters?

·        What % get spirometry?

·        Is there a feedback mechanism for providers to see if pts have picked up medications?

·        ER info

·        Drug info

·        Who is teaching asthma in hospital and ER and what is Grade?

·        What discharge plan do we give people with asthma?

·        How are your providers taught about asthma and how are they given updates? 

·        Any f/u for provider edu? 

·        Any CME requirements for asthma?

·        How are nurses and educators taught about asthma and updated? 

·        Who is evaluating medication use?

·        Where do they go?  What is grade for f/u?

·        Are patients’ QOL evaluated?

·        Is there chart review for pts with asthma (action plan, immunization record, etc)?

·        Quality measures for new pts with asthma?  What will they receive – especially edu?

·        Any staff designated for asthma edu?

·        Who would you give report card to?  Hospital council, CEOs, DPH.  Top down from hospital council.
