SF Asthma Network (SFAN)
|
Maria Palazuelos (Medical Dir. Of Silver Ave. Health Cetner) has a nurse who took and passed the exam. Idea to mentor this individual and support to start working toward SFGH-type model program.
Discussed that KP grant could support 4 sites toward becoming model asthma clinics.
6 sites that could potentially participate including
Silver Ave Health Center & Tom Waddell.
Member suggested a toolkit might be developed to offer to medical directors to create buy in.
Strategy:
Let’s have a sub-committee to increase community involvement with SFAN
1. Look at website and who Medical Directors are
2. ID who is already in Network – approach them to get idea of who to meet with. We want to get more involvement in your clinic, who can we contact?
3. Get a toolkit together containing all materials from previous meetings – handouts, slides,

Epidemiologist working on surveillance with California Breathing state within EHIB which is now part of State DPH (CDC funded – coming to end of 5 year funding and applying for 2 more years of funding). California Asthma Partners (asthmapartners.org) – convenes NGOs throughout the state. Went out to train school nurses on Starlight Starbright toolkit.
Starting a Housing program and granting program. Janet Tobacman has been attending SFATF Environmental Committee meetings.
Asthma Surveillance at state level has grown with CDC funding.
Most local data can get is Hospitalization and ER rates by Zip Code.
CHIS – CA Health Interview Survey
Can get questions put on if there is money to do so. Might want to see if we can encourage CA Endowment to over-evaluate a specific geographic area.
BRFSS – can do the same but Health Depts.; samples are done by people’s homes so shelters and homeless are not counted.
Surveys were done in English, Spanish, Mandarin, Cantonese, Vietnamese, Korean. SF was concerned that phone surveys missed some other language groups.
Can give us rates of hospitalization and ED visits by zip code. Might want to do over a course of a range of years.
OSHPD data is collected yearly.
Could use more questions on how asthma affects day to day life.

Meredith will send copies of state surveillance summary to Anjali. Zip code data as well.
We can contact Meredith with questions related to data or data needs.
Have Meyla request CHKS data by county for SF. Must be specifically requested by County.
Anjali will have a conversation with Meredith re: city/county/zip stats.
