MINUTES

 

Present: Gloria Thornton, Nadia Thind, Marie Hoemke, Louanne Lee, Stephanie Manfre, Anjali Nath

 

I. Clarification on adult asthma staff person at SFGH peds clinic

Discussion:

Andrea Crosby is part of expansion at SEHC. 

Grant to rearrange clinics from acute care to chronic care model.  Depression, backpain and asthma/COPD were main reasons people were coming to clinic for adults. Want to create 4-5 NP positions.  Will hire an NP to be the link from specialty clinic to primary care. Dr. George Su is helping organize asthma/COPD segment.  Still looking for an NP.

Problematic at every level to motivate staff and especially physician staff.

Take standards and guidelines that have been modeled and put it out there as city standards.

 

Processes are slow and we need to keep that in mind.  G. Thornton suggested creating a small committee who can work to get standards set.  Before we meet with medical directors and higher ups we should have something concrete to bring to them.

Suggested meeting with David Nunez, Susan Janson, both health plans and BTMG, M. Drennan or L. Johnson.

If we want to sustain in medical community the medical community in its various pieces needs to take ownership.

 

Idea to get Mitch Katz to write a letter to ask medical directors and medical providers to encourage them to take Asthma CME course. In line with being a model city, be part of an initiative. And ask to have a part-time clinic in each one of the health centers – designate ½ day per week for asthma and SFGH can offer consultation. 

Question if he is the best person to do this?  Maybe he supports someone else to take the lead on this.

 

Make a list of things we would like to see in SF with DPH help.

Convene a smaller group to talk about where we should go (CMEs?, enhancing capacity of clinics?, building leadership of providers?, look at data and find out where worst asthmatics are,

 

¨ Invite Tangerine Brigham to ATF meeting to talk to us about HAP

 

II. SF Asthma Network

¨ Finalize date, speakers and location  for early May (WAD?)

Next meeting would be a half day – Nan (case study examples and people bring their own questions), Shannon Thyne (teaches on diversity issues), Nadine Burke and Helen Lu (working with specific communities, effective communication), Peg (Skin Testing – examples, how to do at clinic), GERD (Oct.), Links with stress and chronic illness and exacerbation of illness.  Discussed how to best have discussions around ethnicity if at all.  May want to think about just having conversation about what exacerbates asthma (violence, no show rates  - impacts on asthma management).

More info on medications

Live spirometry – have everyone practice – might be a waste of time

Demonstrate inhalers

Share handouts – how to send to them, email?

Psycho-social factors

Knowing difference between COPD and asthma

 

Other suggestions for future:

Better communication with speakers and confirmation of details

Loation - Fort Mason Center run by Golden Gate Rec area.

¨ Nan will send info on upcoming speaking engagement - Specialist from Harvard, Dec 13th

 

III.UCSFResearch Request

Could put a link on ATF website or newsletter in interest of building and maintaining relationships.  Find out if unethical to post info about clinical trials? How does Breathe CA do it?  Could put in a disclaimer.  Marie inquired about how communication is going to be facilitated

 

IV.  Additional Meeting Outcomes:

¨ Gloria offered to host an asthma networking meeting in spring or October 2008.

 

¨ Gail will confirm if she can attend November 17th Planning meeting for AAE conference.
