The expansion will cover another 4 to 5 square miles in San Francisco. Work will include system optimization along with system acceptance and testing to ensure that it is in propoer working order.

generate reports on the Outcome and Assessment Information Set (OASIS). The OASIS requirements are a set of rigorous measures that track the sociodemographic, environmental, support system, health status, functional status, and health service utilization characteristics of the patient. The use, collection, encoding, and transmission of OASIS data is mandatory for all home health care patients receiving skilled services. 2) administer the mandated Home Health Consumer Assessment of Health Plan Study (HH CAHPS) by mail to all Health at Home Patients and generate reports required by CMS. The HH CAHPS Survey is designed to objectively and independently measure the experiences of people

receiving home health care from Medicare-certified home health agencies.

Municipal Transportation Agency

Collect, analyze and interpret data on bicycle ridership and user preferences.  This project is a follow up study to a report produced in 2009, which established benchmarks to measure the effectiveness of the multiple bicycle facility improvements, education, and outreach programs implemented by the SFMTA.  The ultimate deliverable of this project will be an independently produced summary "State of Cycling in San Francisco" report that compares current trends to past findings and recommends ways to improve the City's delivery of bicycle programs and infrastructure. The report will analyze bicycling volumes as well as conduct a large-scale telephone and intercept survey to gauge local attitudes toward cycling. The findings of this report are used as performance measures for our major funding source Prop K local transportation sales tax. The report is

also vital to show the general public whether our resources are being used to effectively improve bicycling conditions.
